Prior to accessing forms, read their brief descriptions to determine if you need to complete them. Not all forms are required for each student.

All Regis University forms must be downloaded using Adobe Reader. Please first download this free application before attempting to fill out our forms. If you are still having difficulties, call ITS at 303-458-4050.

FAFSA
To apply for federal, state, and institutional financial aid complete the Free Application for Federal Student Aid (FAFSA) at www.fafsa.ed.gov.  Use School Code 001363 to indicate that the FAFSA Student Aid Report should be distributed to Regis University.  The FAFSA for 2014-15 is the application for summer 2014, fall 2014 and spring 2015. The 2013-14 FAFSA is for summer 2013, fall 2013, and spring 2014.

Colorado Residency Form
Use this form to qualify as a Colorado resident if this information is requested by the Financial Aid office.

Colorado Residency Form - GRADUATE STUDENTS

Authorization to Request COF Stipends 
This form is used to either authorize or rescind authorization for releasing the COF stipends to your student account.

Change of Address, Email Address, or Phone Number

Credit Balance Hold
If a credit balance exists on your account and you would prefer to leave it there for any future charges rather than receive a refund, fill out and submit this form.

FAFSA Income Appeal 2014-15

Use this form to report reductions in family income that are not reflected on your original Free Application for Federal Student Aid (FAFSA).

TEACH Grant Application - 2014-15

For students who commit to teaching under all of the following circumstances:

  1. Full-time
  2. In high-need subject areas
  3. For at least four years
  4. At schools that serve students from low-income families
Graduate students may be eligible for $4,000 per year ($8,000 total) with these same teaching commitments.

Use this form if your 2014-2015 Student Aid Report indicates that you are selected for verification or the Financial Aid office has requested verification.
