Colorado Residency Form
Use this form to qualify as a Colorado resident if this information is requested by the Financial Aid office.

Colorado Residency Form - GRADUATE STUDENTS

Book Voucher Instructions and Form 
Use this form if you have confirmed financial aid and you want to use some of your funds to buy books.

Authorization to Request COF Stipends 
This form is used to either authorize or rescind authorization for releasing the COF stipends to your student account.

Change of Address, Email Address, or Phone Number

Credit Balance Hold
If a credit balance exists on your account and you would prefer to leave it there for any future charges rather than receive a refund, fill out and submit this form.

The Free Application for Federal Student Aid (FAFSA) is available at www.fafsa.ed.gov. Once completed and submitted, this application makes you eligible to apply for federal, state and institutional grants, loans, student employment, and scholarships.

NOTE: If you are using a MAC computer or if you are using Chrome as your browser, you must download Adobe Acrobat in order for many of these forms to process correctly. If you are still having problems, please contact ITS at 303.458.4050.

For Summer 2013, Fall 2013, and Spring 2014

Student Employment Application for 2013-2014
Use this form if you are interested in working on campus. If you fill out the FAFSA, you do not need to fill out this form. This form does not apply to summer employment

Student Employment Application-NEW STUDENTS for 2013-2014
Use this form if you are a Regis applicant interested in working on campus and do not yet have a Regis email address. If you fill out the FAFSA, you do not need to fill out this form. This form does not apply to summer employment.

FAFSA Income Appeal 2013-14
Use this form to report reductions in family income that are not reflected on your original Free Application for Federal Student Aid (FAFSA).

Other Resources Report
If you expect additional funds that are not reported on your Award Letter, such as scholarships and grants, complete this form and submit to the Financial Aid office.

2013-2014 Institutional Verification Form (IVF)
Use this form if your 2013-2014 Student Aid Report indicates that you are selected for verification or the Financial Aid office has requested verification.

2013-2014 Financial Aid Revision 
Use this form to request changes to your enrollment or financial aid award.

Regis College Summer 2013 Financial Aid Application 
Only use this form if you are interested in summer financial aid.

TEACH Grant Application - 2013-14 
For students who commit to teach 1) full-time, 2) in high-need subject areas, 3) for at least four years, 4) at schools that serve students from low-income families. Graduate students may be eligible for $4,000 per year ($8,000 total) with this same commitment to teach.

PT Rotation Appeal for Financial Aid - 2013-14

Intent to Enroll
The Intent to Enroll Form is required if a student has withdrawn from classes and is not attending any other courses in the current term (5-week, 8-week, or variable-length term) but does plan to register and attend at least one course in a future term in the semester. The student should submit the Intent to Enroll form to confirm their future attendance in upcoming terms in the semester. This form is required whether or not the student is officially registered for future courses. If this form is not submitted, a re-calculation and reduction of federal aid eligibility is required. This will result in a reduction in the amount of federal aid available to pay tuition charges and the student will be required to pay this difference. This form allows us to suspend the re-calculation process.

Receive Federal Direct Student Loan Entrance Counseling and Federal Direct Loan Exit Counseling to help you navigate through receiving and paying loans.
